Parents, please sign this form and return it to The Diener School with your completed application. Please make a copy of this release and also send it to your school as well as any therapeutic providers listed in the application

Required Evaluations A full psycho-educational evaluation and report completed within the past 12 months Speech and language assessment Occupational therapy assessment
Please provide any other relevant educational, IQ, therapeutic, neurological or psychological evaluations completed within the past 12 months.
Current Report Card and, if applicable, current IEP Progress reports from all current therapist, counselors, tutors, etc. Confidential Academic Teacher Recommendation
(Confidential between The Diener School and sending School) Teachers completing these forms must have taught the student within the last two years.

Please Note: As we don’t have a recommended IQ score, our students need to demonstrate cognitive potential to benefit from our program.
As we are here to provide the optimal educational experience for our students, we require that all questions must be answered with complete honesty and all of the above documents must be disclosed. The Diener school reserves the right to deny admission or remove for expulsion if there are any material disclosures that were not made.
