This page serves as a checklist, guideline and location for all forms needed to be admitted into The Diener School. Please note that this checklist does NOT save if you click on a link to a new page.

Application Fee
An application processing fee of $100.00 is due at the time the application is returned. This is a non-refundable processing fee. Please make checks payable The Diener School.

Release of Information
Parents, please sign this form and return it to The Diener School with your completed application. Please make a copy of this release and send it to your school as well as any therapeutic providers listed in the application.

Parents, please sign this form and deliver it to the school your child attends. Do not return this form to The Diener School.

Required Evaluations
A Full Psycho-Educational Evaluation and Report Completed within the Last 18 Months, including:

For Grades Early K and K:

  • Cognitive assessment of the Wechsler Intelligence Scale for Children (WISC-IV) or the Wechsler Preschool and Primary Scale of Intelligence (WPPSI) depending on the age of an applicant.
  • Adaptive Behavior Testing
  • And Academic Readiness Testing.

For Grades 1-6:

  • Adaptive Behavior Testing
  • Cognitive assessment of the Wechsler Intelligence Scale for Children (WISC-IV) and
    Achievement or Academic Assessment on the Woodcock Johnson III (WJ-III) or a similar test.

Speech Therapy Evaluation (If an applicant has received speech services within the past 12 months)
Occupational Therapy Evaluation (If an applicant has received occupational therapy services within the past 12 months)
Other Evaluation and Reports Please provide any other relevant educational, IQ, therapeutic, neurological or psychological evaluations completed within the past 12 months.

Please Note: As we are here to provide the optimal educational experience for our students, we require that all questions must be answered with complete honesty and all of the above documents must be disclosed. The Diener school reserves the right to deny admission or remove for expulsion if there are any material disclosures that were not made during the admissions process.
